Please NOTE: Acrobat Reader is required to view forms. If you don't have Acrobat Reader, please take a few moments to download it from the Acrobat button on the bottom of this page.

How do I access forms?
To access the forms, you will need to first log on to the Web site.



Select Forms under the Favorites column.



Select the Carrier, State, and Category. Then click the "Submit Selection" button.


You will see a list of forms similiar to this:



How do I view and print forms?
Choose the form you would like to view by clicking on the PDF link next to the Acrobat icon.

Sometimes the PDF icon will be replaced with the word Original. Original forms must be provided by the carrier. They cannot be downloaded, emailed, or faxed. Add them to your "cart" and they will be mailed to you. To add them to your "cart", select the ADD link on the far right column with the header "Add To Cart".

What does the Add to Cart function do?
Original forms must be provided by the carrier. You can select "Add to Cart" to request the original forms be mailed or faxed. You may also have forms that are not original, but rather, available on demand on the GWINS Web site, emailed to you by adding them to the cart system as well. After selecting the forms and placing them in your cart, select "Check Out" to complete the email/mail/fax request process.

How do I print forms?
Choose the form by clicking the PDF icon.

Once the PDF form is open, there is a printer icon at the top of the PDF frame. Use this button to print PDF documents.

What if forms are not avaliable?
Not all forms will be available online. When you select a carrier, state, and category sometimes you will get the following message:

No Forms Available

This does not mean no forms are available at all. These forms may not be posted on the Web site at this time.

I cannot open an email attachment sent by GWINS? What do I do?
Email attachments sent by GWINS are typically in PDF format. It is required that you have Acrobat Reader software installed to access these attachments. If you do not have Acrobat Reader 5.0, please take a moment to download it from the Acrobat button on the bottom of this page.

If you do have Acrobat, try to save and rename the file to your computer hard drive. Then try opening the file. To do this select File > Save As, and them rename the file something easy. Ex. CaseStatusReport.pdf. Be sure to keep the .pdf file extension when renaming the file.

Then go to where it is saved and double click on the file to open it. If it does not, please contact our Web support.
